Dr. Jacobson has decided to concentrate his efforts on developing and practicing new minimally invasive procedures. These procedures are unusually done through small incisions around the scalp, mouth and ear canals. Minimal incision surgery leads to rapid healing with down times of days instead of weeks. Furthermore, the patients usually experience minimal pain and bruising.

Understanding the Facelift: While a rose by any other names is still a rose - a facelift is a generic description of the surgery of the face.

Facelift

There are many different kinds of facelifts. Choosing the appropriate procedure involves deciding what the patient and the doctor want to accomplish. Pulling skin and muscle does not make you look younger, instead it makes you look artificial.

Facial aging involves changes in facial volumes not just sagging skin. The loss of volume can result in a hollow, tired appearance while the downward redistribution of volume (fat, muscle and skin) causes jowls, neck wattles and bags around the eyes.

Correction of bony and/or soft tissue imperfections that occur with aging are accomplished by adding support and increasing volumes rather than pulling and stretching tissue. This technique known as Facial Recontouring, is a new approach based on sculptural theory, resulting in the most natural "non operated" appearance.

Facial Recontouring procedures involve minimal incisions; fat and soft tissue augmentation; and maximal utilization of the body's available tissues. This technique is the newest most progressive and advanced thinking in modern cosmetic surgery.

Segmental Rejuvenation and Minimally Invasive Procedures:

Current medical thinking is that the face can be divided into ZONES OF AGING.

Zones of Aging are: » Upper third » the brow (forehead)
    » upper eye lids
    » glabella (between eyebrows)

» Middle third » lower eyelids
    » nose
    » cheeks

  » Lower third » lips
    » mouth
    » jowl
    » neck

New Minimally Invasive Techniques allow us to choose a specific zone of aging and selectively address this area. In doing this we are able to rejuvenate the part that bothers us most, avoiding the need for a major surgery, such as a full facelift.

Minimally invasive surgeries provide a rapid recovery of days instead of weeks.

Most procedures are performed under local anesthesia, thus further reducing risk to the patient. On average most patients experience minimal discomfort, minimal bruising, and are able to return to near activities in several days.

These new techniques have changed my thinking to the philosophy that less is more.

Segmental Face Lifts- are minimally invasive procedures which address a particular part of the face or neck.

  • J Lift: Jowl and Neck
  • S Lift: Jowl
  • Neck Lift: Turkey Wattle
  • Soof Lift/midface: Cheeks
  • Brow Lift: Eye brows
  • Piriform Lift: Aging Nose
  • Lip Lift: Elongated Upper Lift
  • Lip Shortening or advancement: Aging Lips

S-Lift- is a technique of minimal incision face lifting - performed under local anesthesia or with IV twilight sedation. Healing time is rapid, allowing the patient to be back to near normal activities in 5-7 days.

Two 1 1/2 inch incisions mostly inside the ear which usually heals without the appearance of any obvious scars. This technique can not affect your hearing and will not interfere with hearing aids. This technique is specific for correction of jowls and upper neck.

J-Lift Procedure - The J-lift is a minimally invasive facial rejuvenation procedure developed by Dr. Alan Jacobson. Similar to the S-lift, the J-lift is performed without the need of general anesthesia, and requires a bandage for only one day. There is usually minimal-to-no bruising. The downtime is a matter of several days. The procedure involves using a small incision which spares the posterior hairline. The jowl and neck are tightened and elevated in a superior vertical vector. The procedure results in a natural, non-operated appearance and is ideal for both men and women. The J-lift is usually preferred over the S-lift in patients who have more pronounced neck laxity and muscular sagging. Most patients are able to return to near-normal activity in about five days.

Neck Lifting - With the availability of new minimal incision surgical techniques, the neck can now be corrected without the need of a standard facelift. Performed under local anesthesia or IV sedation, healing time is rapid and the patient can resume near normal activity in 5-7 days.

Two 1-1 1/2 inch incisions are made behind each ear and one 1 inch incision is made under the chin.

This is a procedure that is very popular with men. Say good bye to your turkey wattle.

Mid Face Lift- is the newest of all the face lifting procedures. The mid face lift us usually accomplished through an incision in the hairline or inside the mouth. Thus, there are no visual scars. Down time is minimal, no more than a few days. The mid face lift is specifically designed to correct the folds between the nose and mouth as well as the loss of cheek fullness. When combined with a lower eye lid procedure, the hollowness, circles, and bags below the eyes can be easily eliminated.

Soof Lift - as we age we all get the appearance of having bags under the lower lids. This is often times associated with a crescent shape fullness on the cheek below the eye and triangular hollow below the eye next to the nose. The soof lift permits correction of these problems through a small inconspicuous incision below the eyelashes or inside the eye lid.

Comprehensive Facelifts - Some are not candidates for minimal invasive procedures. In these people correcting one specific area would only result in an unbalanced disharmonious appearance. For these patients there is an array of different procedures specifically designed to produce the maximum result with the least amount of downtime. None of these surgeries require general anesthesia.

A comprehensive exam is required to determine the appropriate procedure selection.

  • Mini
  • Lowerlift
  • Extended smas lift
  • Deep plane lift
  • Subperiosteal lift
  • Endoscopic lift

Lip Revitalization - As we age the upper and lower lips undergo a significant change. We notice that our upper lip elongates and we seem to lose the pink show of the lip. Vertical lines appear along the lip edge and women noticed increased difficulty in applying lipstick. Similarly, the lower lip looses volume and definition. Fortunately, many new minimally invasive procedures are available to correct and balance the appearance of the lips. The best choice of correction modality lies in the proper analysis of the problem. Fillers such as fat, restylane, perlane, Radiesse™, JUVÉDERM™, ArteFill™, etc., can be injected in the lip area to restore fullness. Similarly, minor surgical procedures can be done to improve the lip line as well as the pink show the lip (Lip Lift).

As an Otolaryngologist and Cosmetic Surgeon, Dr. Jacobson is very aware of the function of the nose as well as the appearance.

The Nose: Nasal surgery can be done with many techniques, both interior and exterior, with minimal bruising and discomfort.

As we age the nose elongates and the nasal tip droops. These problems can be addressed non-surgically with BOTOX® Cosmetic and or injectable fillers. The results are beautiful and there is NO down time.
