You will need to set up your patient profile before we schedule your consultation appointment. To create a secure link to our patient portal, we need your Date of Birth and Zip Code. Please call us or email us to provide us with that information.

We also need a few pictures of your face and a short video summarizing what you expect from your surgery.

Once your patient profile is complete, we can schedule your consultation with Dr. Mardirossian. A consultation fee of $500 will be collected to schedule your appointment.**
